The Community Foundation of Middle Tennessee appreciates the work you have done to improve the communities of Middle Tennessee. Complete the Final Report below. The Community Foundation uses this information to measure its community impact from the funding given to nonprofits and to consider recommendations to allocate future funding. Report Narrative

Share your program accomplishments supported by this grant funding and include in your answers: 

  • Successes and any challenges that were encountered during the course of the program. Explain how this funding allowed you to achieve the goals and objectives outlined in your application and work plan?
  • Were the activities conducted as planned and explain? If not, how did you resolve the changes and explain if there is anything you would do differently to mitigate challenges.

Financial Expenditures

Provide an explanation of grant expenses and income. Have you received other financial support from the community because of this grant, explain? Has your organization made efforts to diversify your revenue stream during the grant cycle and have those efforts been successful? Resulting Benefits and Impact

How would you summarize the overall impact of the program on your clients, staff and agency? Have those you serve requested other services, not previously offered by your organization, in response to this particular program? If so, please describe?

Submit one copy of:

  • any media coverage, press releases, newsletters, copies of social media feeds or other methods used to share information about this award and services available in your community. 
  • any photographs of the program or project with captions (emailed, high-resolution digital photos preferred). Include before and after pictures, especially if this was a capital project.  SHORT Videos are welcome.  Because the photographs could be used in digital and printed materials, have all subjects sign the Media/Photo Release located in the Grantee Toolkit. Photos and videos are optional.
  • To provide the utmost transparency, your Final Report, in part or in full, may be posted on The Community Foundation's website. The information provided on your work and the outcomes measured will keep the public informed about the goals accomplished with the funds from the Discretionary Grant program of The Community Foundation.
